Every empty chair in a dental practice is time that can't be sold twice. Appointment reminders are the cheapest protection a practice has against no-shows and late cancellations, and most practices already send them. The difference between reminders that work and reminders that don't comes down to timing, wording and what happens when a patient replies.
When to send dental reminders
| When | Purpose |
|---|---|
| At booking | Confirmation with date, time and address |
| About a week before | Early reminder, enough time to reschedule and refill the slot |
| The day before | Final reminder with a confirm-or-reschedule option |
| Morning of (optional) | Short nudge for early appointments |
The week-before reminder matters more than it looks: it's the one that gives you time to fill the slot if the patient can't make it.
Which channel
- Text is read quickly and easy to reply to, which makes confirmation simple.
- Email suits longer information such as pre-visit forms and directions.
- Phone calls suit patients who prefer them, and appointments that need a conversation.
Ask patients their preference at booking and record it.

What to leave out
A reminder text can be read by anyone who sees the patient's phone. Keep it to the minimum a patient needs: the practice name, date and time. Leave out treatment details, procedure names, diagnoses and balances. That's both good privacy practice and consistent with the HIPAA principle of sharing only the minimum necessary. Use a HIPAA-appropriate messaging tool and a signed business associate agreement with any vendor that handles patient information.
Handle the replies
Reminders work best when replying actually does something. "Reply R to reschedule" only helps if someone acts on it quickly, or the patient can reschedule without a callback. Many reschedule requests arrive first thing in the morning or after hours, when the front desk is busiest or closed. If they sit unanswered, a reschedule quietly turns into a no-show.
When a patient calls instead of texting, AutomateNexus Voice for dental practices answers in under a second, moves the appointment in your schedule during the call, sends a confirmation text, and releases the old slot so someone else can take it. Our guide to reducing patient no-shows covers the rest of the picture.
Recall reminders
Hygiene recalls fill the schedule months ahead. Send the first recall before the patient is due, follow up once or twice if there's no response, and make booking from the reminder as easy as possible. Recall lists are also where lapsed patients hide, so review them regularly.
Tracking whether reminders work
- Confirmation rate: share of reminders that get a confirm reply.
- No-show rate before and after changes to timing or wording.
- Refill rate: share of cancelled slots that were rebooked.
- Opt-outs: rising opt-outs usually mean too many messages.
Common reminder mistakes
- Only one reminder, sent the day before, leaving no time to refill the slot.
- Reminders from an unrecognised number without the practice name.
- "Reply to reschedule" that nobody monitors.
- Treatment details in the message.
Frequently asked questions
When should dental appointment reminders be sent?
A confirmation at booking, a reminder about a week before, and a final reminder the day before. An optional morning-of reminder helps for early appointments.
Are dental text reminders HIPAA compliant?
They can be, if you use an appropriate messaging tool, have a business associate agreement with the vendor, and keep message content to the minimum necessary, such as date and time only.
What should a dental appointment reminder say?
The practice name, the date and time, and how to confirm or reschedule. Leave out treatment and billing details.
Do appointment reminders reduce no-shows?
They help, especially when patients can easily confirm or reschedule, so freed slots can be offered to other patients.
